The Supportive Services (PSH) survey template is designed to measure the effectiveness of the wraparound services provided alongside Permanent Supportive Housingthe case management, mental health support, life skills training, and community connection that make the difference between housing retention and repeated homelessness.

Permanent Supportive Housing is one of the most evidence-based approaches to ending chronic homelessness. But housing alone is not enough. For individuals with complex histories of trauma, mental illness, substance use, and long-term homelessness, ongoing supportive services are what enable them to maintain their housing and rebuild their lives. Weekly or regular feedback helps programs track service quality in near real-time and respond before small issues become crises.

This template captures participant perspectives on the frequency and quality of check-ins, the helpfulness of case management, access to mental health and substance use support, and whether participants feel supported in building the skills they need to thrive. It also measures whether participants feel connected to their community and respected by their service providers.

Use this template to identify trends in service delivery, flag participants who may need additional support, and demonstrate to funders the ongoing impact of your supportive services.
